Condition of Bicycle / Distinguishing Features: 
New (bought 2013 model in October 2012). Bright neon green, but the body has some specks of red paint. Black handle bars with 3 gears (high end). The front wheel is a "left fork" meaning one fork is missing. It looks like a slightly bigger version of a BMX bike. The cannondale logo is in the middle.

Incident Description: 
I went to a Church retreat at St. Clements (642 W Deming Pl, Chicago, IL ‎) in Lincoln Park. I locked the bike to a pole/sign near the church around 7 PM. When I came out at 11 PM, my bike was gone. I looked around for it and came back the next day as well during daylight and couldn't find it.
